| Evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Requires a bachelor's degree in nursing. Requires Registered Nurse (RN).
